No age for Music

The Pandemic years, horrendous as they were, brought out the best in some. Senior citizens felt the brunt; yet there were many who took to singing, writing and even found creativity in form of short videos, funny or otherwise. Social network media was abuzz and new apps came into vogue zooming in on innovative ways to reach out, work from home and most importantly to learn new things, a new way of life.

It is amazing how many seniors took to karaoke singing popular melodies from yore as groups and multi-location videography added to ambience of amateur productions. Music and videos since pandemic have upsurge not just in numbers, but in quality too.

I was fortunately, ahead of this game as one who just turned 65 around then. Over last decade of so, I had already more than 100 self created music videos, many originals on YouTube, etc. I was also operating from my 1-man music band and home studio. No, I was not even a trained musician or singer, let alone composing par Bollywood songs from scratch to master, which were being heard even by established musicians and technicians.

Important aspect was that after a mighty challenging international career which included CEO and Director positions, it seemed God’s will that all roads were automatically taking me towards my hidden dreams. In Mumbai and Chennai I got to work with great musicians and technicians during my job assignments in fact and learn intricacies of music at A Levels.

Such are ways of the almighty. He would hurt me by taking away job opportunities, but seemed to be balancing all this with utmost happiness as I saw my children drift to success in their respective careers with myself happily surrounded by electronics and wires in my ‘cave’, my home studio.

The pandemic in fact ended my career in water and energy infrastructure consultancy for good, which had already tapered down after a heart bypass in 2017.  It was almost a relief as I had created my little home studio in Delhi, which was actually portable enough to be carried to Africa and other terrains where my projects had taken me to over the decade.

Technology needs constant upgrade, but I would generally have no money to spare as my children too were at peak of education and f in to their respective careers. So compared to other studios, ambience was modest. With consultancy earnings trickling in, I would hire musicians. Discarded Apple computers of children as they rose in office added to upgrade my software capabilities. My studio renovation was all self-made despite my very recent bypass surgery. I knew I had to excel, as the opportunity cost of investments in music was already very high.

So it was truly propulsion and thrust I got from family to pass those out-of-job and circulation times. Some began to call us the Trapp Family from ‘Sound of Music’ as my daughter was my singing diva adding to my own voice in music albums; my son played guitar and gave, often, offensive but useful critique whereas my wife, the English language school teacher would put red marks on lyrics. I created my own videos and had to be politically correct in everything, so it all needed serious research.

Well, I also got directed by the kids to right social media support systems to have my music albums posted on YouTube, SPOTIFY, Gaana and other apps  – all this during the peak of Pandemic. At this time I had so much music on my computer sessions, but event organisers and music companies would look past an “old man” who like so many they came across, was ‘taking out his bhadhaas’! Professional outsourcing was turning out to be too expensive for an entrepreneur without salary!

Very recently I have recorded a few devotional tracks with our largest music company. But until these do actually get released, I am on an edge. It has been lyrics of my own songs, which set me right, away from worries.  That my music and voice are getting better like wine with age can only be God’s doing.

A lesson for all, young or old, from my story would be to believe that it is really God who is guiding us to our destiny in His amazing ways. All we can do is to recognize by having faith in our intuitions as we age. We have to take our risks, compete with our own last work only and treat every day as a day at school, not ‘at work’. We are never old as long as the flame to learn new things and to excel in the small things will lead us to the larger picture.

Entertainment Review: Poisoned: The Dirty Truth About Your Food

Americans are as sure of the safety of their food as we Indians are of our immunity built by eating chaat on the street. But food contamination is real and the impact has been lethal, with e-coli and salmonella poisoning causing more deaths than anyone would believe. Stepfanie Soechtig’s documentary Poisoned: The Truth About Your Food (on Netflix) could put people off meat forever, but, the surprising fact is that the supposedly healthy greens, romaine lettuce and spinach are the bigger culprits, so is the innocuous peanut butter. Cantaloupe, sprouts, cut fruits and packaged salads can be dangerous carriers of pathogens.

Soechtig had earlier examined America’s obesity problem in her film, Fed Up; this one sends up a cautionary warning about the food industry’s apathy towards the health of people. In America, a lawyer like Bill Marler and other activists can challenge the major food production lobbies and force some change. But powerful industrialists also have the clout to block law suits. In India, if hundreds of people died of food poisoning, it would barely cause outrage– we all know of green vegetable grown along the railway tracks using sewer water; the vegetable vendor sprays gutter water over his wares, the pani puri seller’s water is iffy, so is the cleanliness of his hands that he dips into the pot of spiced pani. All our food is sprinkled with coriander, which, if not washed well, could be the cause of severe illness.

Poisoned, based on the book by investigative journalist Jeff Benedict, is a level-headed, scientific analysis of foodborne pathogens, as well as an emotionally moving story of parents who watched their children suffer and die. Out of court settlements include a non disclosure agreement, which one angry and grief-stricken father,  Darin Deteiler, refused and became a lifelong fighter for food safety

As Bill Marler comments, the manufacturers don’t consider food as food  but as a commodity. And where money is concerned, there is no end to human greed.

A smiling scientist is put on the mat when she is forced to admit that she accepts research grants from the very corporations that are guilty of ignoring contaminants in their products, because they have call back insurance. On the other side is a whistle-blower whose testimony sent to prison his arrogant boss, who sent out consignments of peanut butter knowing they were contaminated.

Meat product manufacturers smugly push the responsibility on to the homemaker, who is advised to cook the chicken or beef at a high temperature that destroys bacteria. But would fast food restaurants adhere to the stipulated cooking laws?

As a growing number of people in India suffer from hitherto unheard of food allergies, it is time for us to be vigilant too!

Seven techniques to avoid weight regain

Losing weight is challenging. But as anyone who has ever successfully lost weight knows, it’s avoiding weight re-gain that’s the real challenge.

This is true no matter what method you follow to lose weight. For example, studies show that people who follow very low calorie diets (between 800-1,200 calories per day) regain between 26% and 121% of their lost weight five years after treatment. People who follow behavioural weight management programmes (such as WW, formerly Weight Watchers) regain between 30-35% of their lost weight after one year.

Even people who use weight loss medications, such as Wegovy, are shown to have regained about two-thirds of the weight they lost one year after stopping the drug.

There are many reasons why we regain the weight we lose. First, maintaining weight loss is less rewarding than seeing the number on the scale decrease while you’re losing weight. This makes it hard to maintain motivation and continue looking after your weight.

Second, it’s often difficult to maintain the lifestyle changes we made in order to lose weight – especially if these changes are unrealistic and hard to stick with in the long-term (such as very low-calorie diets or cutting out whole food groups).

Third, weight loss can trigger increased production of hunger hormones – and can even slow your metabolism. These changes can make it difficult to resit overeating and can contribute to weight regain over time.

But while weight regain may be a common experience, that doesn’t mean there aren’t many evidence-backed things you can still do to prevent it in the long run:

  1. Be flexible

It’s important to understand that maintaining a healthy weight will require lifelong management – so having rigid expectations and thinking you’ll always adhere perfectly to your lifestyle changes is unrealistic.

Don’t feel guilty when you have a slip-up. Instead, make plans to get back on track as soon as possible. For example, if you think you may have overeaten on the weekend account for this by adding a couple extra walks into your routine the next week.

Doing this can prevent an “all or nothing” approach to weight management – whereby you feel guilty when you don’t achieve your goals and so instead abandon your efforts.

  1. Plan for disruptions

Recognise that there will be disruptions to your weight management efforts – such as holidays, weddings and birthday parties.

Plan ways to navigate these disruptions successfully. For example, losing a few extra pounds ahead of time may adjust for extra weight that may be gained during these occasions.

Or, if you’re going to a barbecue, bring along a healthier option (such as vegetable skewers) so you have a lower calorie option to choose. Doing this will help you enjoy special occasions with less worry.

  1. Be proud of your achievements

Our weight naturally fluctuates over time – and so being proud of yourself when you achieve your goals, regardless of the number on the scale, is important.

Research also shows that people who focus more on how they can achieve their goals – rather than the outcome – are more likely to stick to behaviours important for maintaining weight loss. This might be because they’re less likely to be affected by setbacks (such as regaining some weight).

  1. Make habits

Creating habits can help maintain weight loss. This is because habits are thought to be less affected by fluctuations in motivation.

This means that even when we can’t be bothered, the habits we implemented to help us lose weight will be easier to stick with when trying to maintain weight loss. You could also create some new habits after losing weight – such as going for a walk after dinner or taking the stairs when possible.

5. Get active

A study which looked at people who successfully maintained their weight loss found that physical activity was the most important factor for keeping weight off. This is because physical activity can offset some of the calories we eat.

The best physical activity for maintaining weight loss is the one you enjoy doing most. This is because you’re more likely to stick with it in the long-term if you enjoy it. But research suggests you should try to get at least 250 minutes of exercise each week to maintain weight loss.

6. Weigh yourself regularly

Weight fluctuates by as much as 1kg-2kg throughout the week. By weighing yourself regularly, you can develop a personalised weight range of your highest and lowest average weight. This will help you to keep track of your weight, and understand whether you need to make any changes to your diet and exercise habits in order to maintain your weight loss.

Research shows that people who use personalised weight ranges are better able to prevent large weight regain because they’re able to adjust their behaviours when necessary.

7. Eat breakfast – and focus on fibre

Although the overall evidence on the importance of breakfast in weight management is mixed, one study found that almost 97% of people who kept their weight off reported having breakfast each day.

Another study also found that people who ate plenty of vegetables and high fibre foods – such as wholemeal breads, brown rice and oats – each day were more likely to avoid weight regain. Eating these types of foods means you feel fuller and are more likely to eat less.

Maintaining weight loss can be hard, but that doesn’t mean it’s impossible. And even if you’re only able to maintain a small amount of the weight you lost, remember it can still be very beneficial to your health.

What do very happy people have in common?

The wall street journal, through a survey followed up with 12% of respondents who called themselves ‘very happy’ and asked them what they were so happy about.

Here are some traits shared —

  • Companionship
  • Being involved in community
  • Enjoying hobbies

We are all very well aware that companionship is always and will always be number -1, a close personal relationship with caring and sharing is what warms the heart and makes life worth the ride. Volunteering work for the community you live ranks high too, we humans love a sense of belonging, a sense of being appreciated. When you feel you are at a stage in your life where being a blessing to others brings joy to the soul, you will set out each day, purposefully looking out for opportunities to be a blessing. There is always a counter reaction to your motives, if they are pure, they will bring contentment and in turn good health of mind and body. But for many coordinating timings and getting together with people does not always work out. The third aspect of enjoying hobbies works well for many. It’s at your time, your pace, and your will. Which really is more comfortable as we turn into older adults.

Do you know that some of these hobbies apart from making you happy can make you live longer?

Here’s the list —

Gardening — it’s a double whammy- enjoyment and moderate physical activity. Besides the early morning sun counts for your daily intake of vitamin D, not to mention the breathing in of oxygen. All the nurturing you do for your saplings and see them grow into plants, bloom and add beauty around you act as stress busters, mood enhancers and reduce the chances of dementia. Making your life worthwhile.

Reading — My father always said —“if you have a book in hand, you will never be lonely.” Though in today’s world you are spoilt for choice with surfing the net, cable T.V. and social media. The concentration to read a book is dwindling, but if this habit has been inculcated in you from a young age, you will not forgo it for anything else. A study from the University of Sussex in England has found that “simply picking up a book and diving deep into it, getting transported into another world can reduce stress levels by 68%. The heart rate slows down in six minutes from the time you begin reading. Now that’s amazing.

Cooking — Studies show that eating home cooked meals at least five times a week could add years to your life. Home cooking is always a healthier option, a less expensive option and therapeutic when you learn the art of rustling up your meals from scratch and dishing them out to look appealing to the eye. The cherry on the icing is sharing it with someone else. Attending a cooking class online or in person may not be such a bad idea after all.

Certain forms of exercising —

Among the multitude of forms of exercise, there are some, which focus purely on fat loss, burning calories, muscle building and there are others which are purely to reduce stress, improve mental health and in turn increase your life span.

Yoga and swimming are two forms of exercising that fall into this category.

– They are known to be beneficial to people of all ages and cause the least amount of injury.

– They stretch the muscles; its low-impact is gentle on the joints,

– They help breathing patterns and improve concentration.

– Another side effect is providing cardiovascular exercise and endurance using your own body strength.

– Yoga in particular, increases body awareness, attention to the senses, and massages the internal organs.

All for, over all health leading to longevity.

Music — This one is closest to my heart, listen to music while you shower, while you exercise, in the background while you work, when you need a good cry. There is music for all reasons and all seasons too. Patrick Fagan- Goldsmiths University, London, found that just twenty minutes of listening to music could boost your sense of wellbeing by up to 21%. “Our research showcases the profound impact concerts have on feelings of health, happiness and wellbeing – with regular attendance being the key.”  So keep listening up to add years to your life.

Meditation — There is scientific evidence, the level of the stress hormone ‘Cortisol’ can be reduced by the practice of meditation. Meditation is an ancient practice that involves quieting of the mind and focusing inwardly for a period of time. With mental health issues on the rise it has gained modern credibility to help people relax, concentrate, reduce stress and boost memory. All these factors contribute to a healthier longer life.

In conclusion —If as an older adult your daily routine is eating, drinking, exercising, I urge you not to fall into this mundane trap. Keep adding to life till your last breathe. That’s a life worth living.

Pep yourself up with short walks

“Depression is an omnipresent mental illness that leaves you feeling empty with bouts of sadness and the inability to derive or feel pleasure from anything.”

The sad truth is that depression rates are at an all time high since the

Covid-19 pandemic and are increasingly prevalent especially among seniors.

Treatments for depression vary based on the intensity of your condition. Antidepressant medication, psychotherapy or even a combination of the two is often prescribed. However, there are certain lifestyle changes and natural ways to get some relief.

Exercising is one such solution, it is good for your health in so many ways.

I know, I know! When you suffer from depression or anxiety, exercising is the last thing you want to do. But do you know, when it comes to depression, exercising has shown to have psychological and physical benefits. It can improve your mood, balance your hormones and reduce physical as well as mental manifestations of anxiety and trust me, once you motivate yourself enough to get moving, it can make the biggest difference.

Walking off your depression symptoms —

Exercise is a powerful depression fighter for many reasons. The most tangible reason being the ‘runners high’ and surge of endorphins you feel after a brisk walk or run. That sense of euphoria is unmatched.

So which form of exercise fights depression the best?

The best answer by a mile is any aerobic exercise, walking being the safest for seniors.

Walking is an aerobic exercise that’s suited for almost everyone, anywhere, anytime. All it takes is a pair of comfortable shoes, motivation and you’re ready to go. The release of endorphins reduces your perception of pain and triggers positive feelings in your body which in turn reduces the intensity of depression symptoms. It also as a form of exercise can decrease muscle tension, calm your nerves, improve your quality of sleep and reduce anxiety.

If depression has made you sedentary —

Start off slowly and gradually build your time and distance.

Set realistic expectations when starting out.

Note: While walking can be a powerful tool for your mental health, it’s not a substitute for treatment by a professional.

Even a little bit of exercising like a short walk is better than nothing —

If you don’t have time for a long walk,

let me rephrase this — if you don’t want to make the time or you just don’t have it in you to walk, even if it is a short walk  — Know that it is an acquired skill and you can train your brain to accept this activity as part of your life.

Here is a perfect example — I personally do not need to be motivated to exercise, but today, as I am penning this article, I am guilty of shamming from my morning yoga routine and short walk. It happens to the best of us, but I have learnt that when in doubt or making a decision on whether to exercise or not, always, always just begin your routine and the rest is history.

It is important to listen to your body too, especially if you feel sick.

Slow it down, shorten the routine until you bounce back. Even a short walk of five to ten minutes is helpful.

It’s a rolling effect —The more you exercise, the more energy you’ll have, the more you’ll be motivated to get moving and the better you will feel.

The key is to commit to it however little it may be. Something as simple as walking can improve the quality of life of a depressed person.

How much is enough?

The U.S. Department of Health and Human Services recommends getting at least 300 minutes of moderately intense physical activity weekly.

Commit to begin with a five-minute walk after each meal, then increase to ten…fifteen minutes, till you have clocked 30-45 minutes daily.

When you break up the walk time, it is easier to accomplish.

This can have a significant and positive effect on your depression symptoms. Smaller amounts of physical activity can also make a difference though it will take longer to notice.

Exercising can be as effective as antidepressants in some cases. The benefits of exercise on mental health are well documented, without any side effects. Reaping the mental health benefits of exercise is easier than you think.

How does exercise help depression and anxiety?

Exercising is also considered to be a treatment for depression and might even work better than “medicine” for depression. A little exercise can go a long way to prevent depression before the symptoms even present itself.

Regular exercising can help depression by —

  • Releasing feel-good endorphins in the brain which enhance your sense of well-being
  • It takes your mind off your worries and serves as a distraction. It helps break the cycle of negative thoughts that are food for depression and anxiety.
  • Exercising gives you some quiet time with yourself and allows you to cope with your depression in a healthy way. Indirect self-love.

Final word — For people suffering from depression, taking the first step is very difficult but once you do, the benefits and effects it will have on you and your life are monumental.

Heart is the engine of the body. Heart is a vital muscular organ which pumps nutrient and oxygen rich blood to the body. It is a pump which never rests.

It beats approximately 70 times a minute; pumping almost 5 L of blood per minute.

The body stops to function if the heart stops for even a few minutes.

Heart itself also requires blood for its proper functioning. And this is supplied by the coronary arteries. If these arteries develop any kind of obstruction or blockage, the supply to the muscles of the heart begins to get hampered.

If the blockage is severe, the blood supply starts reducing and it can cause possible damage to the heart muscles with incomplete perfusion of blood, making the heart weak.

Heart disease has become very common in recent times. There has been an increase in the number of patients coming with heart attacks. Heart attack is the number 1 killer worldwide.

And the most dangerous trend that we are seeing is young adults and people in their 20s reporting to the hospital with heart attacks.

India is already the world capital for diabetes, hypertension and heart disease and the burden is only increasing.

Silent heart attack is a heart attack with few,  if any, symptoms or with symptoms that are not recognised as heart attack.

There are occasionally patients with complaints of acidity, discomfort etc, and when they are evaluated at a later date and followed up by an ECG and 2D ECHO, the investigations are indicative of the individual having suffered a silent heart attack at an earlier date.

Typical heart attack presents as:

  • Chest pain/ discomfort- in the centre/ left side of the chest
  • Breathlessness
  • Profuse sweating
  • Choking in the throat
  • Pain in the jaw/arm

The symptoms can be anywhere from nose to navel.

If you have any of these symptoms which aggravate on activity and are relieved on rest- are symptoms which should not be neglected.

A silent heart attack will present with symptoms which can mimic any other abnormality such as

  • Discomfort in the back
  • Acidity
  • Fatigue and tiredness gets better and you ignore it and continue with your work, with reduced effectiveness.
  • Strained muscles
  • Nausea/ vomiting

A silent heart attack is different from a typical heart attack in the presentation of symptoms, and therefore patients suffering from a silent heart attack can get under-diagnosed.

But a silent heart attack like a typical heart attack can cause possible blockage of blood flow to the heart, damage to the heart and heart muscles.

Everything is the same as a typical heart attack, only the manifestation of the heart attack might be a little more mild and atypical as compared to a typical one.

If you suffer from any symptom which feels atypical or unusual to you, something you. Have not experienced this before, it is advised that you see a physician and seek the necessary medical advise and get an ECG done.

In a silent heart attack, the blockage in the coronary arteries might be very severe, but the symptoms might not give an accurate picture about the severity of the disease.

In patients with uncontrolled diabetes, there is damage to the pain carrying receptors, thereby reducing the sensation of pain, thus the presentation of symptoms and diagnosis of the underlying disease.

Most offend a heart attack, or a Myocardial Infarction, as its called, is due to the deposition and blockage caused due to the plaques making the arteries narrow.

Occasionally, the plaque coated arteries may rupture forming a clot, thereby severely impairing the blood supply to the heart, increasing the load on it for further pumping blood to the rest of the body.

Heart failure is when the heart is still functioning but failing to function optimally.

Early recognition and timely treatment is of prime importance.

Risk factors for a heart attack can be classified into:

  1. Modifiable risk factors
    • Increasing age
    • Gender: males > females (before menopause)
    • Positive family history
  2. Non modifiable risk factors
    • smoking
    • Consumption of alcohol
    • Uncontrolled diabetes/ hypertension
    • Obesity
    • Sedentary lifestyle
    • High cholesterol levels and dyslipidemia- LDL (bad cholesterol) should be low and HDL (good cholesterol) levels should be high
    • Stress

Gastrointestinal Stromal Tumors (GISTs) – The Unique and Mysterious Tumors of the Gut

As we observe Sarcoma Awareness Month, it’s time to explore a lesser-known group of tumors called gastrointestinal stromal tumors (GISTs) that affect the gut. While rare, these tumors can have a significant impact on patients’ lives. Dr Ajesh Raj Saksena, a cancer surgeon at Apollo Cancer Center, Hyderabad, sheds light on their occurrence, symptoms, early detection, evaluation, and the treatment strategies that give hope to those facing these unique challenges.

Dr. Saksena explains, “GISTs are a rare subset of tumors that develop from specialized cells in the gastrointestinal tract known as the interstitial cells of Cajal, which help regulate intestinal motility. Though rare, these tumors can arise anywhere in the gut, including the stomach, small intestine, colon, and esophagus. While they were once elusive, advancements in diagnostic techniques have allowed us to better identify and understand them in recent years.”

Who is at Risk?

While the majority of GISTs appear to arise without a known cause, there are specific elements that might increase the risk:

Age and Gender: GISTs usually affect adults, with most diagnoses occurring between the ages of 50 and 70 years. It is important to remember that these tumors can emerge at any age, and there appears to be a slight male predominance in their occurrence.

Genetic Mutations: Specific genetic mutations, such as those found in the c-kit gene (CD117) and PDGFRA genes, may play a pivotal role in the development of these tumors in about 85-90% of cases. These mutations trigger abnormal cell growth and division, leading to the development of these tumors. More recently, other uncommon contributory pathways have also been identified which are associated with deficiencies in the succinate dehydrogenase complex, alterations in the NF1-gene and BRAF gene.

Family History: In a small percentage of cases, individuals with a family history of GISTs or related conditions, such as Neurofibromatosis Type 1 (NF1) or familial GIST syndrome, may be at an increased risk.

Dr. Saksena emphasizes, “It is crucial to keep in mind that GISTs can develop at any age, and although very rare, we have observed cases even in children and younger individuals. These tumors are driven by mutations in various exons i.e. specific regions within a gene that contains the coding sequence of DNA, of the c-kit and/or PDGFR genes. These mutations not only influence the prognosis but also present an opportunity for targeted treatment approaches.”

Clinical Symptoms – The “warnings” not to be ignored

GISTs often remain silent in their early stages, making them difficult to detect. Dr. Saksena shares, “In the beginning, GISTs may not cause noticeable symptoms. This lack of specific warning signs poses a challenge in their early diagnosis. They may be slow-growing indolent tumors to aggressive malignant cancers that can invade neighboring organs, metastasize to the liver, and recur within the abdomen.”

As GISTs grow, they can lead to various symptoms based on their location and size. Common signs may include persistent abdominal pain, gastrointestinal bleeding resulting in black, tarry stools (melena), and palpable abdominal masses during physical examination. In some cases, patients may experience anemia, nausea, vomiting, or even obstruction of the gastrointestinal tract.

Moreover, the obstructive signs and symptoms of GISTs can be site-specific. For example, an esophageal GIST can cause dysphagia or difficulty swallowing, while a colorectal GIST may lead to constipation. In some cases, a duodenal GIST might result in vomiting or jaundice. Understanding these site-specific symptoms is crucial in the early identification and management of GISTs.

Early detection is vital in effectively managing GISTs. Timely evaluation and a high index of suspicion play a crucial role in identifying these abnormalities. Additionally, individuals with a family history of GISTs or other sarcomas should be vigilant and seek comprehensive screening. Among the main prognostic factors in GIST patients are the tumor’s size, location, and the mitotic rate.

How do we evaluate?

Various imaging studies like ultrasonography, contrast CT scans, and MRI can now help visualize GISTs and provide essential information about their location and size.

Endoscopy, both upper and lower, allows us to directly visualize and assess the tumor. Typically GISTs are suspected when there is a well-defined mass arising from under the mucosal lining of the gut.

An endoscopic ultrasound (EUS) can be useful to clearly define the layer of origin, define the extent of the lesion, differentiate from other lesions which may mimic these tumors and take guided biopsies when required, avoiding a potential spillage of the tumor. Sometimes, further evaluation with a PET scan may be required to evaluate the spread of the disease or assess response to therapy.

A Comprehensive Approach to GIST Treatment

In addressing GISTs, a multifaceted and coordinated approach to treatment is essential. Dr. Saksena emphasizes, “Surgery remains the primary treatment for localized GISTs. The specific location from where these tumors arise and size of the tumor determine the type and extent of surgery required, which may include wedge resection, partial gastrectomy, or small bowel resection. Utmost care is taken to ensure complete removal of the tumors without any spillage, as this significantly impacts the treatment outcome.”

In the realm of GIST treatment, a remarkable breakthrough has been achieved with targeted therapies known as tyrosine kinase inhibitors (TKIs). These therapies are designed to target specific molecular abnormalities within GIST cells. Imatinib, a tablet initially used for treating blood cancers, has demonstrated remarkable effectiveness in managing GISTs. TKIs may be administered as an adjuvant treatment after the complete removal of the tumor, or as a neoadjuvant therapy to downstage tumors before surgery. Moreover, these medications are employed to manage non-resectable tumors, metastatic disease, and even recurrent scenarios. Notably, significant progress has been made with the development of second-line and third-line therapies, offering promising outcomes for patients facing GIST challenges.

Regular follow-ups and imaging studies play a crucial role in monitoring treatment response and detecting any recurrence or metastasis. Dr. Saksena concludes, “By raising awareness about GISTs during this Sarcoma Awareness Month, we can promote early detection, better treatment, and improved outcomes for those facing these unique challenges. Together, we can conquer the these tumors and achieve the best results”

Nearly 1/5th of items sent for recycling, end up in the wrong heap, waste sorting at recycle facilities are not super efficient or accurate either. That’s where CircularNet comes in.

  • USING SUSTAINABLE HARDWARE —

6- PLASTIC POLLUTION THROUGH VISUALIZATION —

We all know that Plastic degrades into smaller pieces called microplastics, which then end up in the air we breathe.

Google Arts & Culture worked with and came up with ways to take action on reduction of microplastics.

Data artist Giorgia Lupi to visualize these particles in a virtual exhibit.

– The interactive Plastic Air experiment, offers a glimpse into the composition of plastics in the air — a mix that could include, granules from bottles, fragments from broken CDs or fibers from polyester textiles.

  • SUPPORTING EACH OTHER TO MAKE THE CHANGE —

There are a number of companies which Google.org supports in their ideas to tackle this humongous challenge of a plastic free environment.

Gringgo is one such company; it uses technology to improve waste management systems by launching a number of apps, including one for waste workers to track the amount and type of waste they collect. In their very first year they were able to improve recycling rates by 35% in their pilot village, Sanur Kaja in Bali.

Azure Alliance is another grantee of Google.org, who have developed the Azure Fighter, an unmanned, fully electric boat that snatches floating plastic debris from harbours, lakes, and waterways. It is a recipient of the AVPN APAC Sustainability Seed Fund.

Google for Startups provides mentoring and tech support to startups focusing on solving sustainability challenges using technology. Right from food wastage (Nuvilab, uses AI to analyze food waste and improve operational efficiency in restaurants across South Korea) to fashion recycling across all boards reuse is promoted.

Sub-inspector Ravi Shankar Tripathi (Varma), just three months into the job, constantly tongue-lashed by his immediate superior Jagdish Sahay (Gopal Datt), writes a letter of resignation. Deeming it too poetic to pass on, the genial constable Sattu Yadav (Yashpal Sharma) tells him to come up with a proper one, when an acid-attack falls into his hands.

Kaalkoot unfolds from the point of view of the surprisingly naïve Ravi Shankar, who is nagged nonstop by his mother (Seema Biswas) to get married. She keeps calling him at work and sending him photos, making her an even more irritatingly ‘filmi’ mother than normal.  Ravi has been influenced by the anti-establishment poetry of his recently deceased father, who is a town celebrity; he is also bitterly affected by the marriage of his sister, forced to wed her molester to escape vicious gossip.

The latest acid-attack victim is Parul (Shweta Tripathi), who is harshly judged as a prostitute because there was a bottle of booze in her bag, and her photo on a sex website. But, her friends say, she is “not that type of girl,” but if she were, the cops wouldn’t even bother about her pain and disfigurement. In Parul’s clearly unhappy household, her father hates his two daughters because of their gender, and pays more attention to his pet dog.

There are other incidents linked to the case that expose the region’s entrenched misogyny.  Why the series is called Kaalkoot (poison) still comes as a shock. In the process of his obsessive investigation of Parul’s case, Ravi is sandbagged into getting engaged with Shivani (Suzanna Mukherjee), but also understanding his own strengths and weaknesses better. If he has to survive in the police force, he has to be the kind of cop who does not need sensitivity training, but also has to be outwardly macho in that environment.

Even though the show (created by Arunabh Kumar and Saxena) does not say anything new about Indian society, is presents protagonist who could bring about change if he is not corrupted or desensitized by the inhumanity around him. Vijay Varma, who has played some really despicable characters in web series, now plays an inherently good man, trying to fit into a system not built for compassion towards the weak. Seema Biwas’s fluttery portrayal of the mother strikes a false note, but the other supporting cast, Yashpal Sharma, Gopal Datt and Shweta Tripathi Sharma are excellent, so the show that could have been depressing is actually curiously uplifting.

Travel is back in full swing, and so is bad behaviour by tourists.

Popular destinations have seen an uptick in incidents involving tourists in recent years. Reports of a man defacing the Colosseum in Rome shows that behaviour has deteriorated even in places that rarely had problems in the past.

What’s behind these abhorrent acts? One answer, my research shows, is social media. Instagram and TikTok have made it easy to find “hidden gem” restaurants and discover new destinations to add to your bucket list. But this democratisation of travel has had other consequences.

Because people now see their social media connections from their home environment travelling in an exotic location, they assume (consciously or not) that behaviour they ordinarily carry out at home is also acceptable in that holiday destination.

This is known as social proof, when we look to the behaviours of others to inform our own actions. People are likely to act more hedonistically while on holiday. Now, travellers also look to social media for proof of how others behave. If their peers from home are throwing caution to the wind while on holiday, this can cause a domino effect of bad behaviour.

I’ve identified other bad travel attitudes and habits that have emerged as a result of social media-driven tourism.

For example, the identifiable victim effect, which explains how people are more likely to sympathise with victims of tragedies when they know who those victims are. Because tourists are often sheltered in hotels and resorts away from local communities, they might (wrongly) think that travelling to a place far from home is an opportunity for consequence-free bad behaviour. They underestimate or ignore the effect their actions can have on locals or the economy.

The Instagram effect

When people travel to a beautiful place, the temptation to post photos and videos to social media is high. But, as I have argued, this creates a cycle that contributes to more self-indulgent travel.

First, tourists see their friends post photos from a place (revealed through geotags). They then want to visit the same places and take the same sorts of photos of themselves there. Eventually they post them on the same social networks where they saw the initial photos.

Being able to travel to and post about visiting the same places as one’s social group or online connections can be a form of social status. But it means that, in some cases, travellers will put more energy into creating content than they will to exploration, discovery or being respectful to local customs.

Hotspots respond

Bali is one destination with a reputation for social media-induced tourism. The photogenic island, replete with yoga retreats, is a huge draw for influencers.

In response to tourist misbehaviour, Bali introduced new guidelines for visitors in June 2023. These include rules about proper behaviour in the sacred temples, around the island and with locals, and respecting the natural environment.

Tourists now need a licence for motorbike rentals, and may not set foot on any mountain or volcano in Bali due to their sacred nature. Travellers must only stay in registered hotels and villas (which will impact a number of Airbnb properties). Bali has introduced a “tourist task force” to enforce the restrictions, through raids and investigations if necessary.

One new guideline is to not act aggressively or use harsh words towards locals, government officials or other tourists both while in Bali, or, notably, online. This speaks to the role of social media as part of the problem when it comes to bad tourist behaviour.

Other destinations have taken similar steps. IcelandHawaiiPalauNew ZealandCosta Rica and others have adopted pledges for visitors to abide by local laws and customs. Campaigns like Switzerland’s No Drama, Austria’s See Vienna – not #Vienna, Finland’s Be more like a Finn and the Netherlands’ How to Amsterdam are aimed at attracting well-behaved tourists.

Where such efforts aren’t successful, some places such as Thailand’s famous Maya Bay have taken it further and fully closed to tourists, at least temporarily.

Travel respectfully

Remember you are a guest of the host communities when you travel. Here are some ways to ensure that you will be asked back.

  1. Do your research

Even if you’re a seasoned traveller, you may not realise the impact your actions have on local communities. But a bit of information – from your own research or provided by local governments – might be enough to help you act more appropriately. Before you go, look up guidelines or background information on local cultural or safety norms.

Whether you agree with the customs or not is irrelevant. If it is a more conservative place than you are used to, you should be mindful of that – unlike the two influencers who were arrested for explicit behaviour in a temple in Bali.

  1. Put down your phone…

Research shows that when travelling, people can become alienated from their surroundings if they are more focused on their devices than the destination.

Often the most memorable travel experiences will be when you have a meaningful connection with someone, or learn something new that you’ve never experienced before. That becomes harder if you’re constantly looking at your phone.

  1. …or use your influence for good

In popular “Instagram v reality” posts, influencers are revealing the huge crowds and queues behind the most Instagrammable locations.

Showing the less-than-glamorous conditions behind those iconic shots could influence your own social media connections to rethink their personal travel motivations – are they just going somewhere to get the perfect selfie? Having more evidence of these conditions circulating online could lead to a larger societal shift away from social media-induced tourism.

If you have the urge to post, try to promote smaller businesses and make sure you are demonstrating proper (and legal) etiquette on your holiday.

It’s in the best interest of you and your co passengers to know what you must avoid eating before a flight.

Airport scenarios are tempting, report three to four hours in advance, the row of restaurants and bright, fluorescent lights of the shops might seem like they’re calling out your name but it’s time to think twice before you head towards them and dig into the food not because you are hungry but rather bored. Know that all of this food is pre-cooked and brought in, only to be electrically reheated and served. While you might survive most, there will be some triggers that can lead to your gut feeling ill.

Here is a list of foods and beverages you should not eat if you are due to fly —

Salty foods —

Airport food or inflight food options don’t put your health or diet first. It’s just trans-fat, prepackaged sodium-laden foods. Know that salty foods cause dehydration and tend to bloat the stomach area. In addition, when the cabin pressure drops inflight, the existing gas in your digestive system expands, leading to more discomfort, heartburn and tummy ache, dehydration, constipation and fatigue.

Digesting food is much harder when you’re thousands of feet above ground level in a pressurised capsule.

Be smart — eat light starting the day before or the morning of your flight to stay hydrated and light in your tummy.

Over-processed snacks and junk food —

If you have a sensitive stomach and you want to avoid digestion issues while flying, stay away from highly processed foods like candy and fast food. These foods can make your digestive system go haywire causing gas build up, bloating and dehydration.

Keeping your in-flight meals simple to avoid discomfort while you fly.

Fast food —

More often than not, we have no option but to eat fast food at airports. It’s also normal to crave junk food when you’re going on vacation. However, fast food is high in salt, sugar and preservatives which can cause bloating, dehydration and acidity while you fly.

Try carrying some healthy food options from home during your next trip to the airport.

Sushi —

This food might come as a surprise to you but sushi before a flight might seem tempting but its best to stay away.

Sushi has the potential risk of causing food poisoning and not the mention the high sodium levels of soy sauce.

Apples —

Apples are one of the healthiest fruits, but because of their high dietary fibre content its hard to digest it. While you fly, as mentioned earlier, it’s harder to digest foods because of cabin pressure which makes apples even more difficult to digest and potentially lead to bloating and constipation.

It’s best to opt for fruits that are gentler on your digestive system.

Beans —

Beans and some legumes like peas and lentils are hard to digest and can cause gas.

When you’re flying, this can intensify and lead to a very uncomfortable flight.

Cruciferous vegetables —

Cruciferous vegetables like bok choy, broccoli, brussels sprouts, cabbage, cauliflower and collard greens are generally good for you, but are hard to digest and can sit in your large intestine for several hours, especially in a pressurised cabin. These veggies can contribute to intestinal gas and bloating in addition to the gas expansion that happens inside your stomach and intestines during flight. This can lead to uncomfortable gas and flatulence.

Opt for lighter greens such spinach, lettuce, or zucchini when you’re travelling next.

Chewing gum —

Chewing gum might help you with nausea and help you pop your ears but the excess air, sugar and artificial sweeteners can cause flatulence and gas buildup.

If you need a mouth freshener opt for a mint, mouthwash or saunf instead.

Spicy food —

While spicy food is loved by many, it can cause heartburn and acidity.

Its best to lay off the spice if you are travelling. You will thank yourself later.

Fizzy drinks —

Flying can cause most of us to feeling gassy and bloated. Adding to this extra gas is not a good idea. Stay away from carbonated drinks before or during a flight to avoid excessive gas, bloating and heartburn. Instead, drink water to stay hydrated.

However, if you’re feeling nauseous or motion sick, carbonated drinks can be helpful.

Coffee —

Coffee is a diuretic and drinking it on a flight can lead to dehydration, not to mention the caffeine can keep you awake when you should be resting and mess with your sleep schedule. Caffeine should be used very limitedly as a way to remedy jet lag because it can make you feel jittery and anxious.

Opt for an herbal tea instead.

Alcohol —

Drinking alcohol on a flight means getting intoxicated faster and dehydrated even faster than that because of cabin pressure. If you cant resist the free glass of wine to take the edge off, remember to hydrate to minimise unfortunate risks.

Its best to avoid alcohol while flying, if you can.

Skip heavy meals and big portion sizes —

Before getting on a plane, avoid filling up on heavy foods. They can upset your stomach and make you feel uncomfortable.

Opt for a satisfying but not overly heavy meal.

The battle of the bulge for all of mankind is here to stay.

We have all experienced hurdles while trying to lose weight , you’re not alone in this boat for sure. Everyone experiences challenges that are specific to their own journey. Those who are successful at achieving their weight loss goals and maintaining it say that again – “and maintaining the weight loss” are the ones who persevered and learnt how to break through these barriers.

When it comes to losing weight, it’s never easy. There will be roadblocks along the way, obstacles you have to encounter, highs and lows, and even plateaus but once you see that tiny glimmer of hope everything becomes worth it.

When it comes to getting rid of the excess pounds on your body, staying consistent, motivating yourself and losing weight in a sustainable way is key.

Sometimes you may even feel you’ve tried every diet and exercise plan and can’t lose weight, know that there may be a psychological or physical block in your way.

The first step is to identify the roadblocks that are hindering your weightloss goals then figuring out the hurdles is half the battle won! Whats left is execution of the plan.

Lets work on this step-by-step —

Identifying your weight loss barriers —

The first step is to look within yourself. Not many of you know this but a lot of the hurdles you face on your weightloss journey are self inflicted. It’s completely normal for most people to experience ups and downs along the way. Recognising these personal obstacles is the first to rise above them. Whether these challenges you are facing are perceived or concrete, they will fall into one of the three categories: physical, environmental or emotional.

Physical barriers– common physical barriers include discomfort, injuries, medical issues, fatigue, stamina and your general physical ability. While these barriers shouldnt be discounted it is possible to work around them.

Environmental barriers- When your surroundings don’t support your healthy exercise plan, it can feel like a tug of war. Some environmental barriers include  limited access to healthy food, no access to exercise facilities, lack of social support or even lack of time because of social, family and professional commitments.

Psychological barriers- The psychology behind weight loss plays a huge role in whether you achieve your goals or not. There are several main psychological roadblocks that work against you in some ways, but it can work for you in others. In order to get past your roadblock, you’ll first need to figure out specifically, what that roadblock is.

Here are some psychological barriers that can be a nuisance to your weightloss journey-

  • Body image issues- If you are less than satisfied with your current body and steering towards unhealthy ways of getting the “perfect body”, chances are you are battling with body image issues. Negative body image and low self-esteem can hinder your weight loss efforts and leave you feeling dissatisfied with whatever progress you have made.
  • Emotional eating- emotional eating can lead to overeating or indulging as a way to cope with your feelings which can sabotage your weight loss efforts.
  • All or nothing thinking- If you practice all-or-nothing thinking, you might struggle to enjoy a “cheat day”. Its hard for you to go back to eating healthy after a small indulgence. Instead of considering it a setback you tend to give up. There is no balance in your journey making it unsustainable.
  • Unrealistic goals- setting overly ambitious goals and now seeing immediate or dramatic results can leave you feeling demotivated.
  • Stress- High levels of stress while trying to lose weight can hinder your progress which can cause more stress and emotional eating, thus creating a vicious cycle.

Here are some ways to overcome weight loss hurdles and win the battle-

  1.  Understand that your body has changed

Your back hurts, your knees ache and you just don’t have the same physical prowess as you used to. As you age, we lose muscle mass and gain fat mass. The solution to this is strength training using weights, resistance bands, doing yoga or pilates. Listen to your body and understand your physical capabilities. Push yourself but not too hard, this will help you avoid injuries. Both men and women start to lose lean body mass as they age. There are many aspects of aging that are out of your control. Keeping this in mind while setting your weight loss goals will help you tremendously.

  1. Stay hydrated

Simple changes can make losing weight much easier. Drinking enough water and staying hydrated helps with better weight loss results. Staying hydrated also avoids you mistaking thirst for hunger.

  1. Choose appropriate physical activity

Dr Nayantara Das is an ER Physician currently practising at Manipal Hospital, Baner, Pune. Having completed her postgraduate training in the field of Emergency Medicine, she has managed the ER of several multispeciality hospitals in Bengaluru and Pune over the past few years. She specialises in all medical and surgical related emergencies with a special interest in the management of trauma and paediatric emergencies.

Myth: ageing is a disease.

Truth: ageing affects the manifestation of certain diseases and in the process of recovery.

Ageing has/ shows certain subtle changes in every system of the body- nervous system, cardiovascular system, Gi system, and so on and so forth. This is not an indicator of deterioration of health but rather a marker to make changes- in our lifestyle and way of treatment as well.

Special considerations in the senior age group:

  • Late presentation/ delayed presentation to the hospital due to increased tolerance to pain and discomfort.
  • Atypical/ subtle presentation
  • Multiple pathologies and added comorbidities
  • Difficulty in extracting an accurate history- due to lacunae at the end of communication, need for a comprehensive and detailed evaluation is necessary.

Common complaints that taken us to the doctor are:

  • Fall
  • Pain in abdomen, acidity, heartburn
  • Constipation
  • Dizziness
  • Joint pain
  • Reduced appetite
  • Diminished vision
  • Generalised weakness

Some diseases, medical concerns and issues which are seen in the emergency often have a simple solution and require minimum intervention. These are:

  • Falls: a fall is never treated as a single entity in an elderly patient. It is treated as a symptom.

30% of falls in the elderly are environmental/ accident related

70% of the falls have an organic cause

Cause and effect of fall are both important. These can be multifactorial, depending on your pre-existing health status, and previously known comorbidities.

Falls need to be paid attention to- if there is a history of recurrent falls, in a short period of time. If the patient does not recall falling/ gives a history of loss of consciousness.

You can perform the “Get up and Go” test on someone who has endured a fall, to evaluate the gravity of the situation and whether the individual needs to tube rushed to the hospital or not?-

  1. Make them sit.
  2. Ask them to get up and walk 3 metres
  3. Take a turn.
  4. Walk 3 metres back and sit down.

While they are performing the test, you need to look out for any peculiar gait, imbalance, feeling of dizziness or unsteadiness.

  • Head injury: in a severe head injury, with laceration or contusion, it is very easy to establish that the patient needs to be taken to a hospital. It is in mild to moderate head injuries, that it is important to know whether the individual requires immediate medical assistance and evaluation or not?

In anyone over the age of 60 years, even a very minor/ trivial head injury, it is advised that you go to your doctor. This is important because the presentation of a brain bleed due to an injury/ fall presents differently in the senior age group as compared to a younger patient- which can be ruled out with the help of a smile NCCT Scan.

  • Fractures: immobilisation, stabilisation and occasionally surgery are the mainstay of treatment
  • Delirium/ acute confusion: a state of mind which is different from the usual, and occurs over a short period of time. When your suspecting delirium, ask the following questions:
  1. Is there any change in the state of consciousness of the patient?
  2. Reduced ability to focus?
  3. Difficulty in understanding the language?
  4. Any change in the orientation to time, place or person?
  5. Duration of symptoms?
  6. Any change in the presentation of the symptoms during the day?

It is important to get a patient with delirium investigated and evaluated, to ascertain the cause. Some common causes that may present as delirium in the elderly are:

  • Infection
  • Metabolic disturbances/ electrolyte imbalance
  • Hypoglycaemia
  • Toxic insults, interaction of 2 drugs
  • Neurological conditions
  • Hypoxia
  • Abdominal pain, acidity, heartburn- though common in all age groups, it is more bothersome and concerning in the elderly.

Acute, persistent pain needs to be evaluated in the elderly. Because a serious pathology can present with mild pain as well.

Do not ever ignore abdominal pain, especially if it is acute in nature.

  • Heart attack- it can present atypically and might not even present as chest pain. In most, the symptoms seen include- sweating, pain in abdomen, breathlessness- isolated or in adjunct with chest pain.

The idea is as audacious as it is preposterous, which is what makes Bawaal worth a look. The promo had intrigued viewers– what is the World War II connection with a family drama?

Nitesh Tiwari working with a story by Ashwiny Iyer Tiwari, equates a man’s coming of age with lessons learnt (or maybe not) from history.  Ajay (Varun Dhawan), popularly known as Ajju Bhaiyya is a history teacher in a Lucknow school, popular not for his teaching (abysmal), but for the flamboyant image he has created for himself. He may not know much about the Nazi era, but he has inadvertently absorbed the Goebbelian dictum about a lie repeated often enough becoming the truth. So, all the self-aggrandizing yarns he has told about himself have made him a hero in his mohalla. Only his parents (Manoj Pahwa-Anjuman Saxena) know what a good-for-nothing wastrel he really is.

To enhance is image further, he marries the rich and pretty Nisha (Jahnvi Kapoor), but she has a health condition, which makes him treat her with deplorable indifference. There is no reason for Nisha to put up with his mistreatment, but the hope that he will change. Or perhaps she believes herself to be “defected goods” as he calls her.

In danger of being suspended for slapping a legislator’s son, Ajay comes up with the crazy idea of going on a WW-II pilgrimage and showing students, via video, what happened back then. He actually spends about five years’ worth of his salary to save a job! And because there was a big budget available to make the film, the history portion is WW-ll, which takes the estranged couple to Europe, instead of, say, the Partition Museum in Punjab, for some national history.

Ignorant and out of his depth away from his usual playing ground, Ajay has to depend on the well-travelled and better educated Nisha for everything from information to food.  His learning to appreciate her is understandable; her not rejecting him for the selfish boor that he is, remains puzzling. The film, at its core, is traditional, believing it to be the woman’s duty to reform the man, at the cost of her own self respect.

The WW-ll tourism takes Ajay and Nisha to Paris, Normandy, Amsterdam, Berlin and Poland, and Tiwari recreates in black-and-white some scenes from that era, though the information he imparts to his eager students back home could have been managed by screening a few well-made Hollywood movies. And what he learns about decency and responsibility, from any ‘sanskari‘ Bollywood social film.

A weird concept without the added attraction of memorable music, Bawaal has to depend on the two stars on whom the entire film is focussed, and they do their bit adequately– if they lack chemistry, it works for this plot. Luckily for them the film is released on OTT (Amazon Prime), so viewers will be more accepting of such an experiment than they might have been in a moviehall.

Do you wake up, empty your bladder, and need to drink your cup of tea, maybe smoke a cigerette or down a litre of water awaiting the much awaited “sensation?” For some this struggle (especially as we add on years) is a reality.

A regular cycle of emptying the bowel can be problematic for some for a multitude of reasons. If you are one of the unfortunate strugglers, this article is just for you.

 Why must you poop? (love this universal slang for emptying your bowel)

Pooping is so important for your digestive health and in turn your overall health. Irregular bowel movements or constipation can cause a lot of discomfort and health issues if it goes unaddressed. While there are common over the counter quick fixes like laxatives, stool softeners, suppositories and fibre supplements- tackling the issue in a natural and sustainable way is always the better option.

Eating fibre-rich foods like fruits, vegetables, probiotics, whole grains, legumes and pulses is good for your gut and promotes healthy, regular bowel movements.

Fibre is key- it increases the amount of stool you pass, it softens your stool, decreases the time you spend on the pot and your dependancy on laxatives.

 How does fibre help you poop?

Dietary fibre is also known as roughage. It’s the part of the food you eat that your body can’t digest and absorb. Fibre passes through your intestines undigested which helps form and soften your stool to pass seamlessly.

There are two types of fibre-

  • Soluble fibre– this type of fibre absorbs water and has a gel-like consistency, which is responsible for softening stool and accelerating its movement through the intestines. Soluble fibre also helps reduce your cholesterol levels and blood sugar levels.
  • Insoluble fibre- this type of fibre passes through your digestive tract as is. Its what helps to add bulk to your stool and presses on the digested food to pass through the intestines easily and frequently.

Including a mix of soluble and insoluble fibre in your diet is recommended to help reduce constipation, bloating, and gas.

Foods that will help your bowel movements —

From the fruit family —

Plums-prunes-Apples-Pears-Figs-Mangoes-Ripe Banana, Grapes-raisins, Kiwi, papaya, avacado.

All of theses fruit contain the natural form of insoluable fibre pectin, sorbitol or enzyme ficain (known for their laxative properties) along with the water content (which dry fruit lack, hence opt for fresh fruit over dry fruit) this together soften your stool and quickens the journey to the exit.

Added information —

– Keep the skin on the fruit whenever possible, it houses the maximum fibre.

– A serving of five prunes contains 4g of fibre in the form of pectin and sorbitol best remedy.

– Pears are high in fibre, sorbitol and fructose which is slowly absorbed in limited amounts by your blood because a large amount of it is metabolised by your liver. The unabsorbed sorbitol and fructose helps loosen your stool.

– Figs contain an enzyme called ficain, which helps with pooping.

Grapes and raisins are fabulous constipation warriors. Full of fibre and water but, high in sugar content, something to be aware of.

The key to eating bananas is to eat the ripe ones. Ripe bananas are packed with  constipation-easing fibre and help you poop whereas unripe bananas contain starch which can actually aggravate constipation. Pick bananas that are fully yellow or slightly browning.

From the vegetable family-

Cabbage, Sauerkraut and kimchi, Leafy greens, ladyfinger(bhindi), Ginger, brocolli, beans

Vegetables even though they dont contain the same amount on insoluable fibres like fruit, they have fibre in bulk and load the pressure on the food in the digestive track to pass quickly through your digestive track.

 Added information —

– Sauerkraut and kimchi are fermented cabbage dishes which are full of probiotics. The probiotics present in these dishes aid boost gut health and can help alleviate constipation issues.

Greens such as spinach, brussels sprouts, kale and arugula are not only rich in fibre but also full of folate, magnesium, vitamin C and vitamin K. These dark and leafy greens help add bulk to your stool which helps it pass through your digestive tract.

– The slimy centre in ladyfinger is premixed water and soluble fibre which softens your stool and helps get rid of constipation issues.

Ginger is a digestive aid which helps ease bloating and constipation.

– Broccoli contains sulforaphane which protects the gut and aids in digestion.

Most varieties of beans have good amounts of soluble and insoluble fibre, which can help you regulate your bowel movements.

From the nut and seed family-

Nuts- almonds, pecans, cashews, peanuts and Seeds- flaxseeds, chia seeds are excellent sources of soluable fibre that promote bowel movement. A must in your daily diet, but make sure you wash them down with alot of water due to its dry consistancy.

– Nuts are also rich in nutrients and minerals.

– Flaxseeds are high in polyunsaturated fats, omega-3 fatty acid and soluble fibres. ground flaxseeds are much better absorbed than the whole seed.

– Chia seeds are not only high in fibre, but they’re also rich in protein as well. It can elevate a low fibre meal to a high fibre one.

From the lentil family —

Green peas, Chickpeas (kabuli channa) Lentils (dal)

Its a double whammy for these foods, plant protien as well insoluable fibre.

 Added information —

– Green Peas contain soluble and insoluble fibre which helps to soften and add bulk to your stool.

Adding a bowl of lentils to your meal increases the  production of butyric acid, a fatty acid found in your colon which helps with digestion and pooping.

 From the oil family —

Olive oil, peppermint oil, avacado oil, flaxseed oil have a mild laxative effect. They coat the walls of the instestines enabling smoother movement in the digestion process.

 Added information —

Olive oil is high in healthy fats good for your heart.

A warm cup of peppermint tea or hot water with a few drops of peppermint oil will help reverse constipation, ease nausea and even alleviate any stomach pains. Peppermint relaxes the muscles along your intestines and allows food to move through faster.

 From the grain family —

Brown rice (husked) oat bran, oat meal,

Choose the husk over refined white rice. It has more fibre and nutrients. The husk adds extra fibre to your diet which pushes down the food through your intestines and out your rectum.

 Added information —

Oat bran is the outer casing of the oat grain. It is richer in fibre than plain oats but oats shouldnt be discounted either. Although oatmeal and oat bran come from the same oat groat, they vary in texture and taste.

 Others —

Yogurt, kefir, coffee, water.

Yogurt is a good source of probiotics, live bacteria and yeasts whereas kefir is a fermented milk beverage that contains probiotics. Both these probiotic substances ease constipation and help regulate your bowel movements.

Even though coffee does not have any fibre it can help with your bowel movements. Caffeine increases gastrointestinal motility by helping your gut muscles contract and notify your brain than you need to use the loo. Dehydration is a common cause of constipation. Water is a natural lubricant that softens the stool and helps resolve constipation symptoms.

 Added information —

– Probiotics are beneficial bacteria that do wonders for your gut health and soften your stool. Incorporate yogurt and kefir in your diet if you struggle with constipation.

A fresh cup of joe might kill two birds with one stone when it comes to starting your morning.

– Lemon water and coconut water also aid in digestion and pooping.

Dr Dilip Nandamuri is a Senior Consultant Physician and Diabetologist at Yashoda Hospitals, Hitec City. He specialises in the treatment and management of Infectious Diseases, Diabetes, Hypertension, High Cholesterol Levels, Lifestyle Diseases, Thyroid Problems, Osteoporosis, and Inflammatory Disorders. He has experience conducting Phase 2-4 clinical trials and is well-known for his initiative and ability to work independently. Diabetic complications are classified into:

  • Acute complications
  1. Hypoglycaemia- for a diabetic low blood sugar levels are taken as RBS below 75 mg/dL
  2. Diabetic ketoacidosis- patients with high, uncontrolled blood sugar levels can land into ketosis leading to multi organ failure and occasionally death
  3. Hyperosmolar non- ketotic state- occurs only in patients with type II diabetes. No ketosis is seen in this state but there are high blood sugar levels, disturbed electrolytes and can lead to multi organ failure
  • Chronic complications

These can further be divided into

  1. Microvascular
  2. Macrovascular

This division is on the basis of the vascularity/ size of the arteries involved.

Macrovascular complications include

  • Coronary Artery Disease- in simple words, patients developing a heart attack
  • Cerebrovascular Disease- stroke in the brain, when blood supply to the brain/ certain parts of the brain will be reduced or completely obstructed leading to paralysis of certain parts of the body depending upon the area involved.
  • Peripheral Artery Disease- compromise in the supply of blood to the peripheral arteries of the body, especially the lower limb- this can lead to diabetic foot ulcers, neuropathic pain, gangrene- this can lead to amputation

Microvascular complications include

  • Diabetic retinopathy- vascular complications leading to diminished vision or complete blindness in the eye
  • Diabetic nephropathy- gradual increase in the protein excretion from the kidneys leading to increased creatinine and urea in the blood, further leading to end stage kidney disease where the patient may require dialysis or even renal transplant

Do cities make people, or is it the other way on the urban merry-go-round? If the first were true, then many of us have a multiple personality. In my case, it’s a secular trimurti. However, mere mortals are less clear-cut than deities. So Shiv (Sena) Mumbai did not quite destroy what Calcutta created, and I remained confident of my paternity during the years I spent in Do-you-know-who-my-father-is? Delhi. I was brought up in what was still Calcutta; the other two contributed to my upward mobility, and, yes, even my downgrading.

Having been back and forth in all three, I can say that the old are becoming as obsolete as landlines. ‘Eentaalectual snobbery, money and power no longer define the people of Kolkata, Mumbai and Delhi respectively, that’s if they ever did. In all three, the elites always had all three. In Calcutta, the bhodrolok, in Bombay the Parsis and in ‘Deh-li’ the lalajis. If these entitled beings have lost their genetic markers, so have their home-cities. Money and power cohabit khullam and khulla in Mumbai and Delhi; while musty ‘Cal’ clubs still cling on to their nostalgia and absurd dress codes, change has finally begun sweeping over this city too.

Consider just one indicator. The café is driving out the coffee house. In my time, even those of us from swanky Loreto College would dress down for a token pilgrimage to College Street and its presiding deity –India Coffee House where the air was turgid with Charminar smoke and leftist polemics, both impenetrable. Now the young sashay along Purna Das road where middle-class ‘baris’ have Cinderella-ed into a myriad cafes cum boutiques, each newer and cuter-named. Like us Loreto-ites, they dress down too—in distressed jeans. Though my jaw dropped to see a PYT in mauve angora shorts. College Street is lined with equally legendary secondhand book stalls. The trendy cafes too are lined with shelves of books, but their patrons prefer to be left to their own ‘devices’.

Thus, Flurys, Park Street’s once-exclusive ‘tea-room’, began offering a dinner menu after this iconic avenue became easily accessible, and brought in patrons who couldn’t make it during leisured-class hours. At one level this altered Kolkata reality truly demands a ‘suspension of disbelief’. But thankfully, the impromptu adda, the pavement chess, the heated debate over chai and shingara are still alive and kicking; strains of Rabindrasangeet still float out of windows to find counterpoint in the conch blown at dusk; and yes, some babu still leaves his soulless municipal cubicle, and goes home to translate Brecht.

Mumbai had been marching to a more frenetic drum beat long before I arrived there at the start of the 70s. Like sex in the notorious ‘cages’ of Foras Road, everything was fatatat; show me the money and take the goods, no time for unproductive chit-chat. The figurative fast lane has got faster — though the concrete one has got slower thanks to the insane construction of transport infrastructure.

In that binary of work and leisure, the Kolkatan has a leisurely approach to anything resembling the former – and a business-like bustle when it comes to office picnics. A Statesman colleague drawled with proud disdain: ‘Over five generations, no one in my family has lifted anything heavier than a pen.’ In stark contrast, when the Mumbai executive isn’t working, she’s networking. A corporate czar said bluntly, ‘If I haven’t initiated at least three contracts at a cocktail, I consider it an evening wasted.’ Which, I guess, is a local version of the social contract.

It can’t be otherwise in a city whose double helix comprises dhandho (business) and its grease rokda (cash). After all, Bombay was born in a dowry chest, and blossomed with the hormones of commerce. Even the serial bomb blasts of 1993 went its economic jugular, bringing 12 of its most arrogant corporate towers to their knees in almost as many minutes.

Its people have also scripted a gladrags to riches story. When we launched Bombay Times and its signature Page Three in 1994, none of us knew how it would hype glamour to unthinkable heights, and launch a million aspirations across the social register. It became the Holy Grail of the middle classes; more incredibly, political Page One People air-dashed to Bombay parties desperately seeking P3P-status. In this disturbing phenomenon, Celebrity has elbowed out Fame; and Achievement languishes a distant third on the roster of goals.

Mum-bais may sweat it out in the commuter cattle trains, chopping dinner vegetables on the exhausted ride home, but Bom-babes inhabit their own metaverse. The pampered, perfumed penthouse parvenu have their own eternal triangle: Willingdon Club, Alibag and Gstaad, — or any ultra-luxe getaway they can get to in their private jets and/or yachts, with matched, monogrammed luggage and designer-dressed nannies in tow. In fact, forget the Goa holiday ‘shack’, the Dubai second home has actually become their first home. Now, thanks to all the raids, the taxman has overtaken the gossip columnist in telling us about their staggering possessions.

Where do Delhiites fit in all this? If for the Mumbaikar, money may equal power, but the former is often pursued without bothering about the latter, it’s quite the opposite for the Dilli Billi, the capital’s fat cats. The city’s hierarchies changed dramatically several decades ago. The dominant Lalajis were first replaced by a diarchy of sarkari satraps and cultural czars and czarinas. Then those from art, fashion, food and foreign-funded NGOs insinuated their way to the top of the social pyramid. These would be derided as the Khan Market gang when Narendra Modi ushered in ‘New India’ – and later proceeded to wipe out large tracts of the old. The Raj from Rajpath and the Mughals from streetnames and even the eponymous Garden at Rashtrapati Bhavan. Fortunately the grand monuments which lend so much space and grace to Delhi have been survived unscathed. So far.

The good thing is that, in its best avatar, Delhiwallas have as much culture as power and money, courtesy the institutions both sarkari and private. And the fact that sprawling homes can showcase the art collection, host the mehfil.

As a domiciled Mumbaikar, dare I say it? Delhites have more style. Living in the capital adds a cachet. The diplomatic circle upping the cuisine is only one example of the advantages of greater exposure (but please discount, the vodka gol-guppa, avocado chaat and edamame samosa) . Even the weather is on its side, the changing seasons allow a sartorial showing off. How civilised it is to sit on sun-warmed lawns in silks and matching shawls.

Kolkata too has a winter but, for the old guard at least, dressing up runs counter to its image of high thinking and low living. Even Delhi netas are nattier. Think the sharply cut Modi-ji and the sloppy Mamata-di. Yes, fashion may have been born, baptised and bloomed in Mumbai, but for most of its people it’s too hot, too sticky or too wet to bother. Also, everything’s too rushed, too far and, increasingly, too dug up. Unlike in Delhi, it’s cool to be casual.

Yes, the distinct lines demarcating the political, financial and cultural capitals are increasingly blurred. Cookie-cutter lifestyles in cookie-cutter cities are an increasing reality. But, thank goodness, old habits die hard, even if old habitats have keeled over. What used to be said about those from Oxford and Cambridge remains true of Delhi and Mumbai. The former act as if they owned the world; those from the latter couldn’t care who owned it. And what of my ‘native place’. ? ‘Owner-shoner? Ki nonshense kotha! Kolkata is the world.’

The urinary tract is one of the systems of the body we take for granted, not giving a second thought to the entire process. It just works in the background, ridding the body of impurities, keep it healthy. Any disruption in the system, however, can result in a multitude of health issues for an individual.

The urinary tract consists of the kidneys, ureters, bladder and urethra. Your kidneys filter your blood, creating urine, which travels through the ureters to the bladder, where it is stored. When the appropriate time comes, the muscles of your bladder contract and urine is excreted from your body through your urethra.

The body uses this urinary system in general and the urinary tract, in particular, to get rid of the bodily wastes and extra fluids. When all the parts of the urinary system are all working in perfect order, normal urination occurs and the body can expel wastes efficiently.

However, anything that affects any of these organs and causes a break in this chain can cause urinary diseases.

The term urinary diseases describe a wide variety of conditions related to filtering and also carrying urine out of the body.

Urinary diseases can affect men, women, and children of every age. Urinary diseases affect specific parts of the body. In females they involve the urinary tract, in men; it affects the reproductive organs as well.

Here are some common types of urinary diseases.

Incontinence

Sudden loss of control over the bladder, causing accidental urine leakage.

Types

Transient: A short episode of loss of bladder control, could be due to medication, age urinary infection or even pregnancy (the uterus exerts pressure on the bladder).

Stress: The most common type, this occurs when there is increased intra pelvic pressure, causing the urine to escape.

In normal situations, the increased pressure causes the pelvic muscles to contract, constricting the urethra and stopping the flow of urine. However, certain conditions like old age, pregnancy, hormonal changes cause the muscles and urethra to weaken, causing incontinence even on sneezing or coughing.

Overflow Incontinence: This happens when the bladder doesn’t empty completely, leaving residual urine in the bladder. It overfills rapidly, leading to incontinence. It’s usually seen as an effect of diseases like diabetes.

Urge incontinence: the need to urinate comes over suddenly and sometimes unexpectedly, and the person isn’t able to control the flow. It’s associated with a pathological change in the bladder or nerve damage.

Mixed: A combination of stress and urge incontinence, this is the most common type seen, especially in seniors and in hospitals.

UTI

One of the most common bladder problems, it affects one in three women across the world; it’s likely either you or someone you know has suffered from this condition at least once.

Urinary infection occurs when bacteria enter the bladder and fester there, and could then travel anywhere in the urinary tract.

Common symptoms are fever often accompanied with chills, increased urination, burning and irritation while passing urine. It can also cause hematuria and abdominal pain.

Contained to the bladder, it’s not a critical condition, but if it spreads to the kidneys, it can cause complications.

Nocturnal urination

A condition causing you to wake up several times a night to urinate, this is usually seen in people over 60 years of age. Another possible cause could be excessive liquid intake, especially before bedtime. Pregnancy is one of the most common reasons for nocturia.

Medical conditions such as diabetes, UTI, bladder inflammation can also aggravate nocturia.

Urinary retention

Urinary retention -when you can’t fully empty your bladder- can cause a persistent feeling like you need to urinate, pain in your lower abdomen, and frequent urination. It can be caused by various factors, such as -neurological factors, infections, bladder muscle dysfunction, obstruction, medications.

Increased urination

Urinating up to 7 times in 24 hours is common, and a normal.

Increased urination – more than 8 to 9 times a day and at least once at night, is a common symptom, affecting a large population.

Reasons include UTI; pregnancy induced hormonal dysfunction, diabetes, certain medicines like diuretics, foods that act like diuretics, such as alcohol and caffeine.

Urgency

Described as a sudden, overwhelming need to urinate, it is often accompanied by burning or pain in the urinary tract. Very often, the person will pass little to no urine, though the urge persists, making it a very uncomfortable condition.

UTI is the most common culprit, followed closely by pregnancy, increased liquid intake, alcohol, pregnancy.

Hematuria

Presence of blood in urine should cause a person to be alarmed, but surprisingly, a majority of patients either ignore it or wait for a second episode before visiting a physician. It’s a symptom that shouldn’t be ignored though, being one of the early and common symptoms of bladder or renal cancer.

BPH (benign prostate hyperplasia)

is an enlarged prostate- a gland that surrounds the male urethra.  BPH is common in older men. It is not directly connected to prostate cancer. Although in some cases, BPH can increase the chances of prostate cancer.

The condition of BPH can cause uncomfortable urinary symptoms such as blocking the urine, heaviness in the bladder, urinary tract, or kidney problems. The symptoms of BPH arise due to the increased pressure on the urethra due to prostatic inflammation.  Kidney stones, narrowing of the urethra, or scarring in the bladder due to previous surgery are also responsible for prostate enlargement.

Ureteral calculi

These are kidney stones that get stuck in one or both ureters. Ureters carry urine from kidneys to the bladder. Ureteral stones can block the flow of the urine and cause severe pain. The larger stones can be problematic as they result in blockage. These stones form due to the chemical imbalance in the urine.

Urinary hesitancy

As the name suggests, it is a condition where people have trouble either in starting or maintaining a urinary flow. Though most common in elderly men, it can affect women too.

BPH, infections, nerve damage, surgeries are some of the causes of hesitancy. It could also be psychological, where a person hesitates to pas s urine in font of others, causing problems in certain situations. This condition is called paruresis. Certain medication, such as anti-allergic, anti-depressants can also cause hesitancy.

Symptoms of urinary disorders vary from person to person and based on the cause of the disorders. Common urinary symptoms include:

  • Abdominal cramping
  • Abdominal, pelvic or back pain that can be severe
  • Hematuria – blood in urine
  • Cloudy urine
  • Fever and chills
  • Foul-smelling urine
  • Frequent or urgent need to urinate
  • General feeling of malaise
  • Leaking of urine
  • Pain during sexual intercourse
  • Pain or burning with urination (dysuria)
  • Unexplained weight loss

Observing your urine-

Keeping an eye on urine can often tell you any condition you might be predisposed to, or suffering from. The color, odor and urine temperature can change if affected by certain conditions, such as –

COLOUR– The standard urine colour, called urochrome, is a pale yellow, in normal conditions and when you’re well hydrated. Certain foods and medicines can change that.

  1. Clear – could indicate overhydration, or even liver disease.
  2. Cloudy urine – Medical conditions like urinary tract infection, some chronic diseases and kidney conditions can result in cloudy urine. In some cases, cloudy urine is another sign of being dehydrated. If you have cloudy urine and you’re pregnant, it could be a sign of a dangerous condition called preeclampsia. Cloudy urine with foam or bubbles (pneumaturia) can be a symptom of serious health conditions, including Crohn’s disease or diverticulitis.
  3. Pink/ red – Foods like beet, medicines such as laxatives or rifampicin (treatment for tuberculosis) cause urine to run red. Hematuria (blood in urine), observed in case of renal calculi, severe UTIs or cancers, can cause the reddish hue too.
  4. Orange – dehydration, chemotherapy drugs, jaundice, can all cause orange urine.
  5. Brown – Usually an indicator of dehydration. Medical conditions like porphyria or liver disease can also result in brown colored urine. Certain medications like metronidazole, certain foods like flava beans can give urine a brown color.
  6. Blue/ green – it’s rare and usually food related- some food dyes, some medicines, multi vitamins.

ODOUR – Urine usually has a very distinct, but mild odor, imparted by the amount and type of toxins excreted in it. Any change in the smell of urine can be indicative of a serious disease.

  1. A strong ammonia like odor means urine which is highly concentrated with waste.
  2. A sweet or fruity smell could mean diabetes, a bacterial infection or presence of amino acids. Portal hypertension, a result of liver disease, also gives a sweetish smell to urine.

URINE TEMPERATURE– Urine, on expulsion, usually feels warm. Excessively warm or hot urine could be a sign of an infection.

In women, warm urine could indicate vaginal infections, post pregnancy injury, STD.

In men, it could be a sign of prostatitis, injuries to urethra, STD, UTI.

Improve your Urological Health

It is important that we treat any disease that cause urinary complaints, as well as take steps to improve and maintain urological health.

Medication: Most diseases respond to medication such as anti-biotics, diuretics and NSAIDs. Do not hesitate to take medicines s prescribed.

Surgery: Some conditions, such as strictures or Calculi, need surgery as part of complete treatment.

Dietary changes – A healthy diet and fluid intake go a long way towards maintaining the urinary system –

  1. Staying hydrated – 1.5 to 2.5 liters of water a day is the recommended quantity for a healthy urinary flow.
  2. Staying within a healthy weight range (avoid obesity) – Obesity can increase the intra-abdominal pressure, causing stress and urge incontinence.
  3. Smoke-free lifestyle – Besides causing one to cough, which often results in urine leakage, Smoke can also irritate the bladder, increasing incontinence.
  4. Limit fluid intake in the nighttime hours – excessive fluid intake at night may trigger nocturnal enuresis.
  5. Limit the amount of salt and caffeine intake- caffeine being a natural diuretic, can result in excessive urination, as well as incontinence.
  6. Salt is known to cause water retention in the body. Expelling the extra fluid can be taxying for the kidneys. Excreting the extra salt is work for the kidneys too, and may result in more water being thrown out of the body, than the actual intake.
  7. Refrain from having carbonated drinks – Carbonated drinks are known to irritate the bladder, triggering urge incontinence.
  8. Eat foods rich in probiotics, especially lactobacillus, which is found in yogurt and kefir. Studies suggest that lactobacillus may be helpful for women with recurrent UTIs.

Urological Health

Lifestyle changes:

  1. Strengthen muscles of the pelvic area with Kegel exercises.
  2. If you use soap in the genital area, use an unscented product made for sensitive skin.
  3. Wear loose, cotton underwear. Consider not wearing underwear to bed to help your genital area stay cooler.
  4. Avoid tight-fitting jeans and leggings.
  5. Try to urinate every 3 to 4 hours and avoid holding your urine when you need to go.
  6. Take warm baths to soothe the sensation of urination.
  7. For women: Urinate before and after sexual activity to decrease risks of a UTI.
  8. Wipe yourself from front to back after going to the bathroom
  9. Avoid douching and vaginal sprays.
  10. Read the side effects list of your birth control before taking it
  11. Exercise and eat a whole-foods diet to maintain a healthy weight for your height, as well as your sugar levels.
  12. Monitor your blood glucose levels
  13. Avoid foods like desserts, breads, and beer that may spike your blood glucose
  14. Consistent diabetes management can prevent diabetic ketoacidosis.

Urinary complaints are common and usually treatable issues, if you pay a little attention to what your body is telling you.

Talk to your doctor about all the symptoms. Consult an urologist if required. If these urinary disorders are left untreated, it can have serious complications.
